All rising 8th grade students from Cario Middle School, Laing Middle School, and Moultrie Middle School will be automatically registered with Wando High School and DO NOT need to fill out any of these forms.

1. Two Proofs of Residence:
   1. Rental Agreement or Mortgage / Lease Contract
   2. Utility Bill
2. Student Social Security Card
3. Legal Guardianship or Custody Papers: Required only in the case where a student under the age of 18 is living with someone other than a natural parent. **Custodial Parent MUST register student.
4. Current South Carolina Immunization Form
5. School Records:
   1. Unofficial or Official Transcript of High School Records
   2. Official Withdrawal from previous school
   3. All Rising 9th Grade Students MUST have proof of 8th Grade completion
6. Birth Certificate
7. These forms filled out. If you do not have access to a printer then these forms will be provided to you when you come into the Guidance Office.
